The JPS Oncology & Infusion Center is home to The Hope Team, a team of health care professionals dedicated to providing cancer patients with the best, most personal care possible.

Now an accredited cancer care facility by the American College of Surgeons, the JPS Oncology & Infusion Center is nationally recognized for providing quality cancer care, especially accruing clinical trials.

Located at 1450 8th Avenue in Fort Worth, less than a mile from JPS Health Network’s main campus, the JPS Oncology & Infusion Center is ready to serve your needs.

Scope of Services 

The center offers state-of-the-art care to patients with cancer and blood disorders. JPS offers laboratory, pharmacy and social services in addition to radiology, chemotherapy and general clinical treatment.

Patients may utilize the center as a gateway to the exceptional care given at John Peter Smith Hospital, our Level I Trauma Center, our women's health center and many more services.

You may reach our team by calling 817-702-8300 from 8 a.m. - 4:30 p.m. Monday through Friday to schedule appointments or consult with staff. You may also contact the team using the MyChart app with any questions regarding your medication, referral, doctor's orders or any other aspect of your care.

On your first visit to our center there are some items that your referring physician needs to provide, and some items that you need to provide. Please review the lists below prior to your first visit.

Patient Checklist: 

  • Photo identification
  • Member card for either your health insurance or Medicaid
    If you do not have third party coverage listed above, you may need the assistance of our eligibility team. Please be able to provide:
    • Social Security card
    • Proof of residence
    • Proof of household income
     
  • Your current medications
  • Any medical records provided to you from your physician, including your diagnosis
     

We will request the following information from your referring physician: 

  • JPS Consultation Form
  • A diagnosis
  • Current treatment status
  • Copies of all x-rays and other films
  • Copies of lab and pathology results

  • Current treatment status

  • Full contact information on the referring physician and/or practice
  • Full contact information on the patient
  • Any additional material you feel is necessary to fully understand each individual's case and provide the most sensitive, accurate care
