Care at the Doorstep: How JPS Brings Healthcare Home

At JPS Health Network, caring for our community means removing barriers to care and meeting patients where they are.

For many older adults and medically vulnerable patients, getting to an appointment can be difficult or impossible. The JPS Provider Home Visits program brings care directly to patients’ homes, helping them stay safe and connected to the care they need.

The program was created to better serve patients whose health needs often became more serious because everyday challenges stood in the way of routine care. Transportation issues with limited mobility, missed medications, and caregiver strain can all lead to worsening health and unnecessary emergency visits.

“Now, with home visits, everything is easier. They come to us, and it’s made such a huge difference in our lives.”

“Many of those challenges could be managed at home,” said Natalie Roye, DNP, AGPCNP-MC, CDCES, JPS Geriatric Nurse Practitioner.

Today, JPS Provider Home Visits delivers clinical care, mobile lab services, medication support, and care coordination directly to patients in their homes. That approach not only makes care more accessible, it gives providers a clearer understanding of what patients are experiencing day to day.

“Home visits allow us to see patients in their everyday environment,” Roye said. “We can identify safety concerns, living conditions, or if they need additional support. These are things you just don’t see in a clinic.”

Belinda Sanchez, JPS Home Visit Coordinator and Medical Assistant, helps make that care possible through close coordination with patients and families.

“I pretty much handle everything from A to Z for our patients,” Sanchez said.

From scheduling appointments and managing medications to coordinating essential equipment, Sanchez helps ensure patients stay connected to the care they need. Sometimes, that support goes even further.

“We don’t just see patients’ charts,” Sanchez said. “We see their homes, their living conditions, and what they’re really dealing with.”

The program’s small, dedicated team creates a highly personal, connected experience for patients and families. Alongside Roye and Sanchez, Carolyn Young, LMSW, JPS Mobile Health Social Worker, helps address needs beyond medical care by connecting patients to resources and support systems that help them remain safe at home.

“It’s about making sure patients aren’t facing things alone,” Young said. “We connect them to the support they need to stay safe in their homes and in their community.”

For families managing complex health needs, that support can be life-changing

“Now, with home visits, everything is easier,” Gonzalez said. “They come to us, and it’s made such a huge difference in our lives.”

The impact goes beyond convenience; it brings a sense of stability and reassurance. Knowing care is consistent and accessible when it’s needed most.

“I can breathe now,” Gonzalez said. “It’s less stressful for both of us.”

Over time, regular visits also build meaningful relationships rooted in trust.

“Many of them feel like family,” Roye said. “Some patients don’t want me to leave because of the connection we’ve built.”

That trust is essential when caring for patients in such a personal setting.

“Empathy and trust are everything. These patients are letting us into their lives and their homes, so we want to make sure they feel truly cared for,” Young said.

By bringing healthcare, guidance, support, and resources directly into the home, JPS is helping patients remain safer; caregivers feel less alone, and families navigate complex health needs with greater confidence. It is one more way JPS continues caring for our community wherever care is needed most.
