The Hope Team at the JPS Center for Cancer Care is dedicated to making your time with us the best, most personal care possible.

You may reach our team by calling 817-852-8300 from 9 a.m.- 5 p.m. Monday through Friday to schedule appointments or consult with staff. You may also email the team at any time at cancer.help@jpshealth.org with any questions regarding your medication, referral, doctor's orders or any other aspect of your care.

On your first visit to our center there are some items that your referring physician needs to provide, and some items that you need to provide. Please review the lists below prior to your first visit.

Patient checklist:

• Photo identification
• Member card for either your health insurance or Medicaid
  If you do not have third party coverage listed above, you may need the assistance of our eligibility team. Please be able to provide:
  • Social Security card
  • Proof of residence
  • Proof of household income
• Your current medications
• Any medical records provided to you from your physician, including your diagnosis

We will request from your referring physician the following information:

• JPS Consultation Form
• A diagnosis
• Current treatment status
• Copies of all x-rays and other films
• Copies of lab and pathology results
