What is JPS Connection?

JPS Connection is a tax-supported medical program offered to eligible Tarrant County residents by JPS Health Network. Eligibility is based on household size and gross monthly income according to the current Federal Poverty Income Levels and proof of current residence in Tarrant County. The program is available for clients that do not have Medicaid, Medicare with prescription benefits or any medical coverage that covers all or part of medical services and prescription pharmaceutical costs.

Eligibility Requirements:

A review of the patient's required documentation is performed on any patient entering or returning to JPS Health Network who has no verifiable third party coverage. The purpose of the interview is to determine if the patient is potentially eligible for a state or federal program, or for the JPS medical program, JPS Connection, to assist them with their medical costs.

To determine eligibility the patient is asked to provide certain documentation. Review the Documentation Requirements (Documentation Requirements in Spanish) for a list of the specific types of documentation required for eligibility.

Medically Indigent Program

Patients who are uninsured or underinsured with gross income above the Federal Poverty Guidelines who have outstanding medical bills and meet the program guidelines can be financially screened for this program. Please call for more information.

How can the patient apply?

A financial screening appointment can be scheduled by calling 817-222-2577. Walk-ins will be worked in if time allows. The patient may also apply through the mail or online. Upon review of the online application additional JPS Health Network eligibility forms will be forwarded via mail or email. Mailed, faxed or email scanned copies of the required documentation along with the application will be accepted.
