Speak to an Enrollment and Eligibility Case Specialist

After your application is filled out and you have provided all of the supporting documents, you need to speak to an enrollment and eligibility specialist. He or she will ensure that JPS has all of the necessary information to process your membership.

If you submitted your application online, an enrollment and eligibility specialist will contact you within 10 days. You can either arrange to submit your supporting documents electronically via fax or email, or you can schedule an appointment to meet in person.

If you printed the application from the web site, call 817-222-2377 to make an appointment to come in. Be sure to bring your completed application and all supporting documents to your appointment.

You are not a member of the assistance program until your application has been approved by a JPS enrollment and eligibility specialist. Your enrollment and eligibility specialist will help you through the application process and make sure you are enrolled in a timely manner.
