Fill out the Application

It’s time to fill out the application for a JPS assistance program.

You can either submit your application online and an enrollment and eligibility specialist will contact you, or print off an application and bring it to your appointment.

If you choose to fill out the online application we recommend the most recent Adobe Acrobat Reader software. Applicants can download and install the software here.
